Community nursing delivered in line with the veteran’s assessed needs, care plan and authorised services.

Appropriate medication-related nursing support and monitoring within the authorised care plan.

Health Monitoring
Ongoing observation and communication about changes in health, wellbeing and support needs.
05COORDINATION
Care Coordination
Working with the veteran, family, referrer and relevant health professionals to support continuity of care.
06AT HOME
Respectful In-home Care
Support delivered with dignity, privacy and respect for each veteran’s preferences and independence.
CLINICAL HOME CARE
Nursing care delivered at home.
Our registered nurses provide high-intensity clinical care, personal support and palliative care according to each veteran’s assessed clinical needs and authorised services.
